Job summary

Health & Human Services Commission (HHSC) in Arlington, TX seeks a Community Care Worker IV to perform advanced case management for STAR Kids and the MDCP waiver programs. The role includes authorizing services, guiding transitions, and coordinating with MCOs and providers.

The position requires strong knowledge of HHSC community programs, excellent communication, and the ability to train staff while working under general supervision.

Qualifications

• High School Diploma or GED required.
• Knowledge of managed care programs, policies and procedures.
• Ability to use Outlook, Excel and Word; familiarity with TIERS and SAS.

Responsibilities

• Administers STAR Kids and MDCP waivers; authorizes and notifies on approvals/denials.
• Leads transitions from nursing homes to community-based care and cross‑program coordination.
• Attends meetings, trains staff, prepares reports and monitors service delivery.
• Develops and coordinates waiver explanations, eligibility validation and enrollment.
• Assists with fair hearings and manages MDCP interest lists.
• Provides policy interpretation and supervision when needed.
