Job summary

Catholic Charities of the Archdiocese of Chicago is seeking a Care Coordinator to conduct timely assessments for elderly and frail clients in community settings, develop care plans, and provide ongoing case management. Travel within assigned communities and collaboration with internal teams are required.

The role requires a BA/BS in a related field, strong communication and organizational skills, data entry ability, and CP software knowledge.

Qualifications

Bachelor's Degree in Arts/Sciences in a related field; entry level.
Strong organization, time management and confidentiality.
Excellent communication, problem solving and critical thinking.
Data entry skills; CP software knowledge required.

Responsibilities

Engage and assess the complex needs of elderly, frail and clients in community settings; gather information and develop care plans.
Conduct assessments for program participants; provide case management and develop well written care plans.
Make referrals, monitor services, maintain client files and attend trainings.
Collaborate with internal stakeholders and team members as needed.
Travel independently throughout assigned communities to fulfill duties.
Engage in ongoing learning and apply feedback to improve performance.
