Catholic Charities of the Archdiocese of Chicago is seeking a Community Care Coordinator to perform annual resident assessments, develop service plans, coordinate referrals, and connect residents with community resources to support well-being.

The role collaborates with multidisciplinary teams, delivers educational and wellness programs, handles crises with sensitivity, and helps place residents in appropriate housing while continuously learning to improve care.

Formación

• AA degree in a related field; 1–3 years relevant experience.
• BA/BS in a related field; 3–5 years relevant experience.

Responsabilidades

• Conduct annual assessments of residents' needs and develop annual service plans.
• Research and maintain a network of community service providers for referrals.
• Facilitate educational and wellness programs for residents and families.
• Manage resident and visitor complaints and crisis situations per policies.
• Coordinate annual building assessments to identify frail or at-risk residents.
