CVCAA is seeking a Community Care Coordination Case Manager in Texas to assess needs, develop recovery plans, and coordinate housing, financial assistance, and mental health services for disaster-affected families.

You will liaison with local, state, and federal agencies, nonprofits, and community organizations, monitor progress, and advocate for clients in high-stress environments. Bilingual candidates are a plus; trauma-informed care is essential.

Qualifications

• Preferred degree in social services or related field.
• Minimum 2 years of case management experience, preferably in disaster response.
• Knowledge of trauma-informed care principles.
• Familiar with FEMA or Red Cross programs.

Community Care Coordination Case Manager

Key Responsibilities

• Conduct intake assessments to determine immediate and long-term needs, especially in times of disaster-affected families.
• Develop individualized recovery plans in collaboration with clients, incorporating housing, financial assistance, mental health services, and other supports.
• Serve as a liaison between families and local/state/federal agencies, nonprofits, and community organizations.
• Monitor progress and adjust recovery plans as needed, maintaining regular contact with clients.
• Advocate for clients’ needs within the broader social service system, especially during times of disaster.
• Maintain accurate and confidential case documentation in compliance with organizational and legal standards.
• Participate in disaster preparedness planning and community outreach efforts.
• Collaborate with multidisciplinary teams including emergency responders, social workers, and mental health professionals.
• Provide culturally competent and trauma-informed care to diverse populations.

Qualifications

• Preferred: Bachelor’s degree in Social Work, Human Services, Emergency Management, or related field.
• Minimum 2 years of case management experience, preferably in disaster response or crisis intervention.
• Strong understanding of trauma-informed care principles.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work in high-stress environments and adapt to rapidly changing situations.
• Familiarity with local and federal disaster relief programs (e.g., FEMA, Red Cross).
• Bilingual skills a plus.
