Job summary

Sea Mar Community Health Centers is seeking a Behavioral Health Specialist to provide primary community support and case management for mentally ill, homeless, and substance abuse clients. You will coordinate services and work as part of an interdisciplinary team to develop client service plans and referrals.

Qualifications include an Associate’s degree in Behavioral Health or related field, with eligibility for Agency Affiliated Counselor Registration and a valid license within 90 days.

Qualifications

• Associate’s degree in Behavioral Health or related field with two years of related work experience.
• Eligible for Agency Affiliated Counselor Registration.
• Active Agency Affiliated license within 90 days of hire.

Responsibilities

• Provide care appropriate to age of patients served and knowledge of growth and development.
• Conduct intake screenings and develop client service plans.
• Make referrals to appropriate agencies and assist client self-referral.
• Maintain documentation and participate in interdisciplinary team reviews.
• Maintain confidentiality and report concerns as mandated.
