Job summary

CarePlus New Jersey is seeking a Lifeset Specialist to join a team supporting 17-22 year-olds transitioning to adulthood within a community-based program in Northern New Jersey. You will work with 8-10 young adults, helping them achieve housing, employment, and education goals.

The role requires a bachelor's degree in social services with related experience; a valid driver’s license is required. The position involves on-site work across multiple counties, with trainings and on-call duties as

Qualifications

• Bachelor's degree in social services with one year of related experience in counseling and/or case management.
• Master's degree in social work, psychology, or a related field preferred.
• Valid driver's license and must be insurable under Agency automobile insurance.
• Ability to maintain a flexible schedule.

Responsibilities

• Provide individual services to 8-10 young adults in the home and community setting to assist with the transition to adulthood.
• Meet at least once per week face-to-face with the young person and provide additional support via phone, text, or in person as needed.
• Directly responsible for all interventions for a specific young adult and any additional resources or supports.
• Support a young person in identifying goals and working with them to achieve them.
• Conduct ongoing assessment of the young person's needs and develop service plans monthly.
• Document all service contacts within required timelines.
• Be on-call rotation and respond in person when necessary.
• Attend trainings and supervision sessions.
• Develop and follow up on feedback and recommendations from supervisors.
• Work will be in Bergen, Passaic, Hudson, Morris, and Essex County.
