Job Overview

The LifeSet Specialist is responsible for providing services to address the needs of young people 17‑21 who have transitioned out of care and need individualized skill development and support to transition successfully into adulthood. Responsibilities include helping the young people obtain and maintain safe and suitable housing, continue their education, learn independent living skills, obtain and maintain employment, build a support system, and remain free from legal involvement.

Responsibilities
  • Provide individual services to 8‑10 young adults in home and community settings, meeting face‑to‑face at least once per week and offering additional support via phone, text, or in person as needed.
  • Conduct case management, collaborate with external stakeholders (probation, child welfare, therapists, etc.), and develop monthly service plans from a strength‑focused, solution‑based perspective.
  • Document all service contacts and attend all meetings related to the young adult and/or family to gather information for intervention development.
  • Operate a 24/7 on‑call schedule (rotating on‑call at night and weekends) and respond in person when required by the Team Supervisor, Clinical Consultant, or Program Director.
  • Participate in supervision sessions and trainings, and follow up on feedback and recommendations from the Team Supervisor, Program Director, and Clinical Consultant.
  • Adhere to all ethical and legal guidelines, including confidentiality and mandated reporting.
Skills

Candidates should possess strong organizational skills and attention to detail, flexibility, the ability to manage multiple priorities simultaneously, and excellent written and verbal communication abilities.

Qualifications

Master’s degree in social work, psychology, or a related field preferred. A bachelor’s degree in social services with one year of related experience in counseling and or case management is required.
