Job summary

Behavioral Health Network (BHN) is seeking an Outreach Worker to support children and families in community settings, offering strength-based guidance and helping youth develop essential life skills. You will collaborate with clinical staff to coordinate care and participate in multidisciplinary teams, ensuring that planning is inclusive and responsive to each family’s needs.

You will drive transportation and engagement efforts, document progress daily, and contribute to crisis intervention

Qualifications

• High School Diploma required; Bachelor’s Degree in the human service field, or in absence of Bachelor’s Degree five years related experience working with children and families.
• Individuals with lived experience in supporting a family member with mental illness or disabilities or with personal lived experience preferred.
• Strong verbal and written communication skills.

Responsibilities

• Offers structured one-to-one strength-based and resiliency building activities to the identified child or youth in community settings.
• Teaches social skills, anger management, and other related skills to youth.
• Provides transportation as needed to address care coordination and treatment needs.
• Supports family in accessing crisis intervention services as needed.
• Closely collaborates with program clinical staff to ensure coordination of care.
• Participates in multidisciplinary team meetings and collaborates with all behavioral health providers involved with the child to support goals of the service.
• Coaches and supports the youth in expressing themselves in planning meetings.
• Provides daily documentation of interactions with the identified individual served, specific to the treatment plan.
