Job summary

Behavioral Health Network, Inc (BHN) in Western Massachusetts is looking for passionate staff to provide direct, community-based support for individuals with substance use and mental health needs, helping them access resources and pursue recovery goals.

You will coordinate care with clinicians, act as the primary contact on your caseload, and assist with transportation to appointments and resource access, including housing and financial assistance.

Qualifications

• High School Diploma or GED is required.
• Strong understanding of addiction and recovery is required.
• 1-2 years of work experience in a related behavioral health, substance use, and/or related human services field is preferred.
• Valid driver’s license is required.

Responsibilities

• Provides direct, community-based support to individuals with substance abuse and mental health needs, helping them access resources, build independent living skills, and work toward their personal recovery goals.
• Provides care management support to individuals that promotes improved health, independence, and self-sufficiency.
• Acts as primary “point of contact” for all individuals served on caseload, ensuring their needs are met.
• Includes all appropriate people in the individual’s care including primary care physician, nurses, mental health and substance use providers, and natural supports as identified by the member, with their written consent.
• Transports individuals to appointments and to obtain resources need to be successful in recovery journey.
• Demonstrates working knowledge of community services and how to help individuals access them including health insurance, food and nutritional assistance, housing placement and stabilization, financial assistance, utility assistance, educational/vocational supports and legal assistance.
