Job summary

Somerset County in New Jersey is seeking an Intensive In-Community Case Manager (ICM) on a part-time basis to support the Crisis Intervention Specialist’s service plan for youth and families. The role involves behavioral support, case management, school involvement, and community engagement to address truancy.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Marriage and Family Therapy, Counseling or related field.
• Minimum of one year experience working with at-risk youth.
• Ability to work with diverse populations and be sensitive to ethnic and cultural differences.
• Demonstrates good communication skills including the ability to express oneself verbally and in writing.
• Must possess a valid NJ Driver’s License.
• Must read, write and speak English.

Responsibilities

• Provide minimum two hours of direct support to each child weekly.
• Coordinate with the Crisis Intervention Specialist and schools, police, and other providers.
• Complete all necessary paperwork regularly and in a timely manner.
• Assist with ensuring compliance with court orders and scheduling evaluations/appointments.
• Lead or participate in treatment/service planning meetings as needed.
• Participate in weekly supervision with the Clinical Supervisor and weekly staff meetings.
• Conduct community outreach to schools, police and other agencies.
• Stay informed on developments around school refusal and truancy.
