Job summary

National Youth Advocate Program (NYAP) in Cleveland, Ohio seeks a full-time Therapist to join our Community Based Team, delivering individual, group, and family therapy to youth and families, and coordinating care in the community.

You will travel daily, maintain documentation, meet productivity goals, and collaborate with foster families, schools, and partners, while upholding equity and trauma-informed care.

Qualifications

• Master’s Degree in Social Work or comparable Human Services field from an accredited institution.
• 2 years of work experience working in direct service with youth and families strongly preferred.
• A valid advanced professional license as required and acceptable by state or contract regulations.
• Must have a valid driver’s license, reliable transportation, automotive insurance, and a good driving record.
• A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served.
• Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers.
• Capacity to remain objective and professional in all areas of job function
• Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds.
• Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.

Responsibilities

• Provides individual, group, and family mental health services, case management, transportation, and other professional services.
• Completes all required documentation in a timely manner.
• Travels daily to provide community based services to youth and families.
• Consistently achieves established productivity thresholds.
• Coordinates and monitors services for youth and family in the community.
• Reviews and modifies youth plans to ensure progress and outcomes.
• Establishes and maintains relationships with foster parents and families of origin.
• Assists in family reunification and/or stabilization efforts.
• Provides safety assessment, crisis response, and behavioral stabilization as required.
• Participates in Continuous Quality Improvement activities monthly/quarterly.
• Participates in weekly supervision.
