Job summary

Bond Health Staffing in Brooklyn, NY is seeking a compassionate Registered Nurse to support individuals with IDD and severe mental illness across crisis respite, intensive case management, and community transition services.

You will conduct nursing assessments, manage medications, provide wound and diabetes care, and coordinate with a multidisciplinary team to promote safety, health, and community integration.

Qualifications

• Current NY RN license in good standing.
• Minimum of two years of nursing experience.
• Experience with IDD, behavioral health, crisis care preferred.
• Knowledge of OPWDD/OMH/DOH regulations preferred.
• Strong assessment, crisis intervention, and care coordination skills.
• Ability to work independently and with a multidisciplinary team.

Responsibilities

• Screen individuals for program admission during intake.
• Conduct initial and ongoing nursing assessments.
• Oversee medication administration and monitor outcomes.
• Provide direct nursing care including wound care and diabetes management.
• Coordinate hospital discharges and community reintegration.
• Train Direct Support Professionals on health procedures.
• Maintain accurate clinical documentation.
