Job summary

Jewish Board is seeking an RN to join the ACT team, providing treatment for clients with SPMI/SUD in the community across the city. You will perform initial nursing assessments, ongoing health monitoring, and coordinate referrals with psychiatrists and PNPs.

80% fieldwork with shelters and homes, supervised by Nursing leadership. Responsibilities include creating treatment plans, administering medications, educating clients on health conditions, and serving as a liaison between care providers

Qualifications

• RN or BSN with New York State registration.
• Experience in residential or mental/behavioral health preferred.
• Comfortable with field work and community-based care.

Responsibilities

• Carry a caseload and complete initial and ongoing nursing assessments.
• Conduct weekly visits for clients in shelters or the community.
• Develop treatment plans with the team and educate on conditions and symptoms.
• Administer medications and monitor for side effects per orders.
