Job summary

Access, Inc. in New York State is seeking a Registered Nurse to join our Assertive Community Treatment (ACT) team.

The RN will provide psychiatric and medical health assessments and treatment to program participants, delivering services in the community and in participant homes across Rockland County. The role involves education, advocacy, and collaboration with an interdisciplinary team to implement treatment plans, manage medications, and ensure high-quality care following ACT guidelines.

Qualifications

• Current NYS license and registration as a Registered Nurse required.
• Valid NYS driver’s license required.
• Electronic health reporting experience strongly preferred.
• Prior experience in mental health or psychiatric nursing preferred.
• Bilingual Spanish, Haitian Creole, or Yiddish speaking a plus.

Responsibilities

• Provide psychiatric and medical health assessments, education, and treatment to ACT participants.
• Manage psychopharmacological treatment under ACT MD direction.
• Carry a small caseload and provide case management services.
• Deliver nursing care per agency policies and ACT guidelines.
• Develop and lead health education groups (e.g., diabetes, heart disease, substance use, asthma, nutrition).
• Escort participants to medical appointments when needed.
• Document services and progress notes in the EHR system.
• Participate in interdisciplinary team meetings and clinical supervision.
