Job summary

Trilogy is seeking a Recovery Counselor in Chicago, IL to assess and support clients through home, community, and office visits. Responsibilities include building recovery plans, coordinating care, documenting progress, and ensuring crisis coverage.

A strong emphasis on trauma-informed, strengths-based care and harm-reduction approaches is required. Earn $22.89–$26.32 per hour with a $1,500 sign-on bonus and a Mon-Fri 8:00am–4:30pm schedule.

Qualifications

• Bachelor's degree or higher in counseling/related field required.
• Licensed Practical Nurse under Nurse Practice Act is acceptable.
• CRSS or IAODAPCA certifications preferred.
• IM+CANS certification preferred and may be required within 30 days.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and support system.
• Complete daily progress notes per Trilogy documentation policy and agency standards.
• Document all client care in the EMR and related forms (IMCANS, LOCUS).
• Identify opportunities to move clients to appropriate care levels; provide referrals as needed.
• Assist clients in recognizing crisis signs and coordinating care or stabilization as required.
• Provide psychoeducation and medication training per Trilogy policy.
• Coordinate with nursing/pharmacy to ensure med accuracy and MAR documentation.
• Help clients learn independent living skills (hygiene, nutrition, budgeting).
• Assist with entitlements applications (SSI, SSDI, Medicare/Medicaid, LINK).
• Support client money management per agency policy and provide transport to appointments.
