Job summary

Trilogy is seeking a Recovery Counselor in Chicago, IL to assess clients and plan treatment through home, community, and office visits. You will offer motivational, strengths-based, trauma-informed services for serious mental illness with co-occurring disorders, using harm reduction.

The role emphasizes case management, documentation, crisis coverage, and collaboration with a multidisciplinary team. Must have relevant clinical credentials and a valid Illinois driver’s license.

Qualifications

• Must have one of the listed degree or credential requirements in mental health settings.
• Must hold valid Illinois driver's license with daily vehicle access.
• Ability to work evenings and weekends as directed.
• Experience in community-based services preferred.
• IM+CANS certification preferred and may be required within 30 days.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and care team.
• Complete daily progress notes and documentation in EMR per policy.
• Provide crisis coverage as assigned and participate in team meetings.
• Assist clients in learning independent living skills and applying for entitlements.
