Job summary

Trilogy, Inc. in Chicago is seeking a Recovery Counselor to assess clients’ personal, medical, emotional, social, and environmental needs and plan treatment across home, community, and office visits.

The role focuses on advocacy, trauma‑informed, strengths‑based, and harm‑reduction approaches, performing case management and crisis/on‑call duties while linking clients to entitlements and community services.

Qualifications

• Bachelor's degree in counseling and guidance, social work, psychology, or related field.
• CRSS certification through IAODAPCA or similar credential may be required.
• Licensed or certified professional credentials related to mental health are preferred.

Responsibilities

• Independently assess risk and client’s need for higher level of care; i.e. hospital, detox program, emergency room, etc.
• Determine associated risk and coordinate support from colleagues to manage risk and ensure client safety.
• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Implement engagement strategies to bring clients into services and assist with linkage to treatment.
• Participate in initial engagement with clients through street, shelter, jail, and community outreach in Chicago.
• Complete intake paperwork and coordinate with referrals (e.g., DOC, shelters, hospitals).
• Conduct mental health assessments and determine service needs and level of care.
• Coordinate authorization for services with managed care organizations.
• Establish an individual recovery plan with client and support system.
• Provide referrals to additional services as needed and assist with housing, budgeting, and social support.
• Educate and assist with applying for entitlements (SSI, SSDI, Medicare, Medicaid, SNAP).
• Accompany clients to appointments and support housing, substance use treatment, and medications.
