Job summary

Trilogy Careers is seeking a Recovery Counselor in Chicago to assess clients’ overall situation and plan treatment through home, community, and office visits. You’ll provide motivational, client-centered services for individuals with severe mental illness and co-occurring disorders using harm-reduction approaches.

The role emphasizes community-based services, documentation in MAR/EMR, and collaboration with a multidisciplinary team.

Qualifications

• Bachelor's degree in counseling or related human service field.
• LPN license under Nurse Practice Act (IL).
• CRSS or related rehabilitation credentials are preferred.
• Two years of supervised clinical experience acceptable in lieu of degree.
• High school diploma or GED with 5 years of supervised clinical experience.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation during visits.
• Coordinate development of individual recovery plans with client and care team.
• Complete daily progress notes in EMR according to policy and standards.
• Provide crisis/on-call coverage as directed.
• Assist clients in learning independent living skills (hygiene, budgeting, nutrition).
• Educate clients on entitlements (SSI, SSDI, Medicare, Medicaid).
• Advocate for clients and link to community services.
