High Point & Affiliated Organizations seeks a Recovery Coach to support patients with substance use and mental health recovery in Brockton, MA. This full-time, on-site role emphasizes compassionate peer support, collaboration with an interdisciplinary team, and mobile services as needed.

Responsibilities include providing emotional support, guiding recovery planning, connecting members to treatment and community resources, documenting engagements in the EHR, and maintaining productivity.

Qualifications

• High school diploma and lived experience with substance use and recovery.
• Excellent verbal and written communication abilities.
• Ability to collaborate with a diverse interdisciplinary team.
• Proficient with computer software and EHR systems.
• Familiar with HIPAA and confidentiality requirements.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide emotional and social support to Members.
• Assist Members in developing recovery plans and skills.
• Help Members connect to treatment, peer recovery and community resources.
• Document patient engagements in the EHR system and maintain productivity.
• Work mobile and on-site to support families and individuals as needed.
