Job summary

Apis Services, Inc. seeks a Psychiatric Rehabilitation Counselor to deliver community-based mental health services to adults with serious mental illness.

Under supervision, you will help clients build independent living skills and access resources while contributing to care planning and crisis response. You will work with an interdisciplinary team to implement rehabilitation plans, document progress in electronic records, and support vocational and social goals in diverse community settings

Qualifications

• Bachelor’s degree in a behavioral health or closely related field.
• Two years of professional experience in community behavioral health (if no healing profession degree).
• Valid driver’s license with acceptable driving record.
• CPRP or equivalent credential preferred.

Responsibilities

• Provide at least five hours per day of direct, face-to-face psychiatric rehabilitation services.
• Document services, progress, and observations in a timely manner.
• Assist in developing and revising individualized rehabilitation plans.
• Collaborate with interdisciplinary team to monitor mental health status and goals.
• Deliver skill-building interventions for independence and community integration.
• Conduct functional assessments and provide training and supervision as needed.
• Support life skills for successful community living.
• Respond to behavioral health emergencies with crisis intervention.
• Coordinate care with treatment teams and community providers.
• Build relationships with community agencies and vocational resources.
• Interview consumers to gather background and social needs.
• Coordinate vocational training and recovery supports.
• Facilitate consumer groups and psychoeducational activities.
• Provide transportation to appointments and resources as required.
• Review assessments to identify service needs and referrals.
• Maintain accurate clinical records and program documentation.
• Participate in committees, meetings, and quality initiatives.
• Perform other duties as assigned.
