Job summary

Horizon House, Inc. in Delaware County, PA seeks a dedicated Full Time Registered Nurse for our Forensic Assertive Community Treatment (FACT) team.

The RN will conduct psychiatric and physical health assessments, administer medications with the psychiatrist, and provide a range of treatment, rehabilitation, and support services. Under supervision of the ACT team, you will coordinate case management, develop individualized treatment plans, monitor progress, educate families, and respond to crisis

Qualifications

• Board Certified
• Licensed Registered Nurse (RN)

Responsibilities

• Conduct psychiatric assessments and assess physical health needs.
• Provide pharmacologic management and administer medications with the psychiatrist.
• Deliver treatment, rehabilitation, and support services; share shift-management responsibilities.
• Develop, write, implement, evaluate, and revise treatment goals with clients and the ITT.
• Coordinate case management and educate/support families.
