St. Luke’s University Health Network in Pottstown, PA is seeking a Registered Nurse ACT to join an Assertive Community Treatment Team.

The RN ACT provides in‑home and community nursing care, supports symptom management, and coordinates medical and psychiatric care for adults served by the program. Applicants must be a PA‑licensed RN with psychiatric nursing experience, BLS and CPR certification, and a valid driver’s license.

Qualifications

• Active nursing license in Pennsylvania.
• Experience in psychiatric nursing preferred.
• BLS and CPR certifications required.

Responsibilities

• Meet ACT participants in homes or community settings to deliver nursing care and support.
• Provide medication service and observation as directed by team physicians and nurses.
• Coordinate treatment goals, provide counseling for symptom management, and maintain documentation in the EHR.
• Provide on-call crisis services on a rotating schedule and support weekend/evening coverage.
