Job summary

Unison Health seeks a Peer Support Specialist for our Assertive Community Treatment (ACT) program. The role centers on providing peer-led support, developing treatment plans, and helping clients access resources for independent living.

The position requires a high school diploma (bachelor’s preferred), a CPI/CPR certification within six months of hire, and a flexible schedule including evenings and weekends. Equal opportunity employer.

Qualifications

• High school diploma required; bachelor’s degree preferred.
• Must become a certified peer supporter within six months of hire and obtain CPI/CPR after hire.
• Must be at least 21 years old and understand mental illness and recovery processes.

Responsibilities

• Provide peer support in line with policies and standards.
• Assess client needs and help implement treatment plans.
• Document services, monitor progress, and follow up on care.
• Support clients in accessing resources and building independent living skills.
• Advocate for clients and educate families on wellness.
• Use lived experience to inspire hope and support recovery.
• Apply client-centered, trauma-informed care and collaborate with teams.
• Manage crises appropriately and maintain professional standards.
