Job summary

Carolina Outreach in Supply, NC is seeking a full-time Peer Support Specialist to join the Assertive Community Treatment (ACT) team. You will provide essential expertise to promote client-centered care and support symptom stability, quality of life, and independence within communities.

Responsibilities include coordinating services with the ACT team, advocating for clients in the community, maintaining accurate documentation, and ensuring compliance with clinical and regulatory standards.

Qualifications

• Must be a Certified Peer Support Specialist (CPSS) in North Carolina.
• Personal experience with mental health and/or substance use recovery is required, with the ability to share lived experiences as part of client support.
• Strong communication and advocacy skills.
• Commitment to upholding the principles of the ACT model and person-centered care.

Responsibilities

• Collaborate with the ACT team leader to monitor clients’ clinical status and responses to treatment.
• Assist in identifying, planning, scheduling, and implementing services tailored to each client’s needs.
• Act as an advocate for clients in the community, supporting their access to necessary resources and opportunities.
• Assist in the development of transition plans when clients move in or out of services.
• Maintain accurate records, including treatment notes, goals, and input into person-centered plans.
• Ensure adherence to the ACT model, as measured by TMACT standards.
• Provide consultation to the ACT team to promote a person-centered approach to care.
• Be available to respond to clients and co-workers as needed, including 24/7/365 on-call support.
