Community Access is seeking a Peer Specialist to join the Bronx Shelter ACT Team. The role focuses on engaging participants with serious mental health conditions, helping them access housing, services, and recovery supports in the community.

The ideal candidate has lived experience in the mental health system, demonstrates flexibility, and can work across Boroughs using assertive engagement strategies to support individuals’ goals and well-being.

Qualifications

• Experience with mental health recovery-oriented services and person-centered care.
• Commitment to serving homeless individuals and people with mental health needs.
• New York Certified Peer Specialist certification (NYCPS) or provisional NYCPS required for starting April 2024.
• Lived experience with the mental health system and preference for those with related histories.

Responsibilities

• Engage participants and build trusting relationships within a multidisciplinary team.
• Assist participants in accessing housing, health, education, and employment supports.
• Document assessments, service plans, and contacts; maintain timely progress notes.
