Job summary

Trilogy, Inc. in Chicago is seeking a Peer Support to assess clients across home, community, and office visits, and to plan individualized treatment through supportive community-based services.

You will provide patient-centered, trauma-informed support and connect clients with community resources to aid in recovery from mental illness and co-occurring disorders. Strong advocacy and documentation skills are essential.

Qualifications

• Must have HS diploma or GED and two years of supervised clinical experience in mental health.
• Must be a past or present consumer of mental health services and willing to share recovery story.
• Valid Illinois driver’s license with daily access to a vehicle and $100,000/$300,000 liability insurance.
• IM+CANS certification preferred and may be required within 30 days of hire.

Responsibilities

• Assess clients’ personal, medical, emotional, social, and environmental situations across home, community, and office visits.
• Coordinate recovery plan with client, support system, and other care providers.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and documentation per policy.
