Job summary

Trilogy in Chicago, IL, is seeking a Peer Support Specialist to assess clients and plan treatment through home, community, and office visits. You will provide motivation, link clients to services, and deliver trauma-informed, strengths-based care to individuals with severe mental illness.

The role includes crisis coverage, case management within a shared caseload, and collaboration with a multidisciplinary team.

Qualifications

• Must have one of the following requirements: High School Diploma or GED and two years of supervised clinical experience in a mental health setting, or an Associate’s or Bachelor’s degree.
• Valid Illinois driver’s license with daily access to a well-maintained vehicle and liability insurance.
• Experience with mental health and/or substance use disorders preferred.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the recovery plan with client, support system, and care providers.
• Model communication, recovery-oriented living, coping skills, and self-help strategies.
• Provide crisis coverage and participate in team meetings to support a shared caseload.
• Complete progress notes timely and document care in MAR/EMR systems.
• Educate clients about entitlements and assist with housing, budgeting, and transportation needs.
• Assist with community referrals and advocacy to empower clients.
