Job summary

Trilogy Behavioral Healthcare is seeking a Peer Support to assess clients across home, community, and office settings to plan treatment and recovery. You will advocate for clients, link to community services, and deliver strengths-based, trauma-informed care for those with serious mental illness and co-occurring disorders.

Responsibilities include coordinating recovery plans, providing psychoeducation, and ensuring timely notes and MAR documentation.

Qualifications

• High School Diploma or GED with two years of supervised clinical mental health experience.
• An Associate’s or Bachelor’s degree in any field.
• Valid Illinois driver’s license with daily vehicle access and insurer coverage.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and care providers.
• Model communication and recovery-oriented living skills for clients.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and documentation (e.g., IMCANS, LOCUS, MAR).
