Job summary

TRILOGY BEHAVIORAL HEALTHCARE is seeking a Recovery Counselor to assess and plan treatment across home, community, and office settings. You will advocate for clients, connect them with services, and deliver trauma-informed, strengths-based care to individuals with serious mental illness and co-occurring substance use disorders.

Qualifications

• Bachelor’s degree in counseling/related human services field.
• Two years supervised clinical experience in mental health setting if no related degree.
• LPN license under Nurse Practice Act.
• Certificate of Psychiatric Rehabilitation from DHS IAODAPCA.
• CRSS certification through IAODAPCA.
• Family Partnership Professional IAODAPCA.
• OT assistant license with mental health experience.
• High school diploma/GED with five years supervised clinical experience.

Responsibilities

• Independently assess risk and need for higher level of care.
• Coordinate support from colleagues to manage risk (e.g., two-person visits).
• Assess client situation via home, community, and office visits.
• Implement assertive engagement strategies to link clients to services.
• Engage clients through street, shelter, jail, and community outreach in Chicago.
• Complete intake for referrals (IDOC, shelters, hospitals).
• Document client encounters in tracking systems.
• Ensure service authorization with managed care organizations.
• Establish individual recovery plan with client and support system.
• Refer clients to additional services as needed.
• Identify signs of decompensation and teach coping skills.
• Help clients learn independent living skills (hygiene, budgeting).
• Assist with entitlements applications (SSI, SSDI, Medicare, Medicaid, SNAP).
• Accompany clients to appointments and support housing, budgeting, and meds.
• Advocate for clients and navigate criminal justice housing opportunities.
• Collaborate with partners for multidisciplinary care.
• Perform other duties as assigned.
