Job summary

Trilogy is seeking a Peer Support to assess clients’ personal, medical, emotional, social, and environmental needs and plan treatment through home, community, and office visits. You will advocate for clients, link them to services, and provide trauma-informed, strengths-based support for those with severe mental illness and co-occurring substance use disorders.

Qualifications

• Eligible to work in a mental health setting and interact with clients.
• Willing to share personal recovery story with clients.
• Valid Illinois driver’s license with daily vehicle access and $100k/$300k liability insurance.
• IM+CANS certification preferred; may be required within 30 days of hire.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental needs via home, community, and office visits.
• Develop an individual recovery plan with client and support system.
• Provide support with recovery, communication, coping skills, and self-help strategies.
• Attend Peer Support meetings and collaborate with the care team.
• Document care and progress in real time per policy and standards.
• Provide crisis/on-call coverage as directed.
