Job summary

Assets, Inc. is seeking a full-time Employment Case Manager to lead service coordination and support for individuals with developmental disabilities/mental illness in community-based employment settings.

The ECM will mentor staff, develop ISPs, and liaise with employers to secure job placements and ongoing support. The role includes preparing plans, tracking outcomes, supervising staff, and ensuring compliance with company policies and budgets.

Qualifications

• High School Diploma or GED required; associate degree preferred.
• Two years' experience providing direct services and case management.
• Must have insured vehicle, Alaska driver's license, and pass a background check; National Certificate in Employment Services or obtain within 6 months.

Responsibilities

• Provide service oversight and coordination for persons supported by the team.
• Coordinate off-site activities necessary for job retention and transportation.
• Conduct face-to-face presentations with prospective employers and act as a consultant to employers in the community.
• Develop and implement Individual Support Plans (ISP) for ongoing support and data collection.
• Complete and submit required documents and reports (Plans, Time Sheets, DVR reports, etc.).
