Job summary

Eliot Community Human Services is seeking a Community Supports Case Manager for its Developmental Disabilities division. The role focuses on assessing needs, coordinating services, and advocating for individuals to access housing, employment, and community activities.

The ideal candidate will have experience with adults who have developmental or brain injuries, maintain a client caseload, and hold a valid driver’s license with access to a reliable vehicle.

Qualifications

• Bachelor’s degree or equivalent experience.
• Experience working with intellectually disabled or brain injured adults.
• Ability to manage a client caseload in an outreach setting.
• Valid driver’s license and a reliable vehicle.
• Ability to work a community based position that services the Lawrence, Lowell, Burlington and Arlington areas.

Responsibilities

• Assist in completing assessments and ISP/POC for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards.
• Ensure individuals are involved in identifying and developing their own goals.
• Document daily progress notes for all individuals in accordance with Eliot Policy and DDS/MRC documentation and billing requirements.
• Develop and maintain a positive and professional working relationship with individuals, families and providers, including timely communication and follow up.
• Coordinate service delivery for cases assigned (case conferences, Section 8 Housing, SSI) and advocate for the consumer with outside agencies.
• Provide service linkages to other providers as necessary and schedule appointments.
