Job summary

OneWell Health Care is seeking a dedicated Caregiver/Direct Support Professional (DSP) to provide person-centered, community-based support to individuals with IDD and Autism in Connecticut. You will assist with daily living, health monitoring, and community participation while following ISP and BSP plans.

Training and supervision are provided as needed. Responsibilities include documentation accuracy, collaboration with teams, and transportation support when required.

Qualifications

• High School Diploma or GED required.
• Experience with IDD or Autism care preferred.
• Experience with behavioral support or community-based services preferred.
• Ability to follow written service plans and protocols.
• English proficiency required; bilingual a plus.

Responsibilities

• Provide person-centered support to individuals with IDD and Autism.
• Assist with daily living activities and skill development.
• Administer medications if certified; monitor health and safety.
• Maintain accurate documentation and records (notes, MARs, incident reports).
• Collaborate with interdisciplinary teams and participate in trainings.
