Qualifications

• High school diploma is required.
• Experience working with individuals with mental health diagnoses or developmental disabilities or substance use disorders.

Responsibilities

• Assess individuals to identify hopes, dreams and supports.
• Help individuals develop community relationships and natural supports.
• Assist to participate fully in the community according to interests.
• Protect rights and support advocacy and personal advocates.
• Provide emotional, physical, and medical support as needed.
• Document progress and complete required records.
