Qualifications

• High School Diploma is required.
• Valid Driver's License (USA) is required.
• Experience working with people who have mental health diagnoses or developmental disabilities is preferred, not required.

Responsibilities

• Assess each person’s hopes, dreams, and desires and provide supports to facilitate achievement.
• Help individuals develop relationships in their community and with natural supports.
• Assist individuals in participating fully in their community aligned with their interests.
• Protect and promote the rights of individuals at all times.
• Support advocacy efforts and utilize personal advocates as needed.
• Provide emotional, physical, and medical support as required for each person.
• Foster person-centered, positive relationships that promote self-determination.
• Input and recommendations into assessments, planning, and development of individual plans.
• Implement plans, document activities, and complete required records and progress notes.
• Assist with day-to-day home operations and transportation as needed.
• Attend meetings and training; maintain certifications and compliance with policies.
