Job summary

Poppy's Therapeutic Corner is seeking a Community-Based Clinical Intake Specialist to join as a 1099 Independent Contractor. The role involves conducting diagnostic assessments, delivering clinical diagnoses, and developing treatment plans for clients at office locations or via virtual platforms.

Assessments may occur in person, virtually, at our offices, or in the community. Ideal candidates are licensed or eligible interns with strong skills in diagnosis, formulation, documentation, and client

Qualifications

• Active Ohio License (LPC, LPCC, LSW, LISW) OR current counseling/social work intern invited to apply.
• Experience with diagnostic assessments, treatment planning or evaluations preferred.
• Strong communication and documentation skills.

Responsibilities

• Conduct comprehensive diagnostic assessments for clients.
• Provide clinical diagnoses and develop individualized treatment plans following Ohio guidelines.
• Complete assessments in person, virtually, or in community settings and document in the EHR.
