Qualifikationen

• Bachelor's or master's degree required.
• At least five years of paid supervised experience with individuals with SMI/SED in case management functions such as assessment, care plan development and maintenance, referral and appointment scheduling, monitoring and follow-up.
• Valid driver's license and completion of the defensive driving course during new hire orientation.

• Provide individuals opportunities to manage their own recovery with support from an interdisciplinary team.
• Conduct comprehensive needs assessments, develop and implement service plans, and leverage referrals to access needed services.
• Assist with crisis response; case management is community-based and primarily home and community-based with client travel.
