Job summary

Residential Transitions Inc. seeks a Targeted Case Manager for Dakota County to assess needs, create service plans, and connect adults with serious mental illness to essential resources.

You will document care, coordinate with teams, and support self-directed living within community and residential settings. Ideal candidates have a behavioral science degree or equivalent, with MN credentials or CMA/RN experience, plus knowledge of person-centered planning and harm reduction.

Qualifications

• Bachelor’s degree or equivalent qualifications in behavioral sciences.
• Demonstrated knowledge of person-centered planning and harm reduction.
• Valid driver's license and access to reliable vehicle.

Responsibilities

• Assess needs, identify goals, and plan services for individuals with SMI and co-occurring conditions.
• Refer and link individuals to needed services and resources.
• Monitor progress and coordinate ongoing services.
• Document services in records with proper standards.
• Provide flexible, individualized support for self-directed living.
• Coordinate with team and stakeholders for service direction.
• Conduct crisis assessment and intervention as needed.
• Promote inclusive, diverse, culturally competent workplace.
• Meet with adults at least every 6 months or as needed.
• Manage civil commitments and court reports.

Education

Bachelor's degree in behavioral science or related field

Meets Minnesota Mental Health Practitioner criteria (MN Statute 245I.04, Subd. 4)

Four years CMA experience (if no behavioral-science degree)

RN with psychiatric training and 3 years MH experience

Completed 80 hours of SPMI training

Demonstrated competency with adults with SPMI
