Job summary

Horizon House, Inc. is seeking staff to provide engagement and case management support within the Philadelphia community. You will conduct home-based visits and collaborate with a multidisciplinary team to move participants toward long-term housing.

The role requires a bachelor’s degree in a related field and at least 2 years of mental health direct care experience, with strong communication and crisis intervention skills. A valid driver's license is required.

Qualifications

• Bachelors or better in Counseling Psychology or related field.
• Bachelors or better in Criminal Justice or related field.
• Bachelors or better in Psychology or related field.
• Bachelors or better in Social Work or related field.
• Bachelors or better in Sociology or related field.
• 2 years of mental health direct care experience
• Ability to communicate with all levels of the behavioral health system
• Valid driver’s license with acceptable driving record
• Fee-for-service and electronic health record experience preferred

Responsibilities

• Provide engagement and case management support, including home-based visits in the Philadelphia community.
• Coordinate with a multidisciplinary team to deliver individualized services toward housing stability.
• Ensure services comply with contract and agency regulations.
