Job summary

Qualifications

• Bachelor’s degree in Human Services or related field or equivalent experience.
• Experience with individuals with developmental disabilities preferred.
• Strong written and spoken English skills required.

Responsibilities

• Coordinate care and provide support in education and social skills.
• Provide meal planning and grocery shopping assistance.
• Assist with personal care and daily living activities as needed.
• Support housing, money management, and community participation.
• Transport individuals to medical appointments and follow up on care plans.
• Educate families and staff on self-directed care and problem-solving.
