Job summary

Qualifications

Responsibilities

• Aids clients in their own home and in the community to promote safety and independence.
• Facilitates physical, recreational, social and educational activities.
• Provides clients with a positive role model and connects them with community resources.
• Maintains liaison with other agencies, professionals, and community stakeholders.
• Monitors clients’ wellbeing and assists with daily life skills and transportation.
