Job summary

Qualifications

• Role involves providing Community Living Supports (CLS) in home and community settings.

Responsibilities

• Provide Community Living Supports to the individual to promote independence.
• Develop daily living skills and increase community participation.
• Support the Individual Service Plan (ISP) goals and ensure safety, privacy, and dignity.
• Work one-on-one with the individual in home and community environments.
