Qualifications

• Bachelor's degree preferred in Mental Health, Psychology, or Sociology.
• Minimum: High School Diploma, CNA, or associate's degree in a social service-related field.
• Valid driver's license and ability to transport individuals in a 9-12 passenger van.
• Pass criminal background check and complete required trainings.

Responsibilities

• Supervise individuals and monitor self-administration of medications.
• Educate on medications and assist with pickups and payments.
• Lead community skills meetings and document daily progress toward goals.
• Transport to day services and medical/psychiatric appointments.
• Assist with daily living activities, housekeeping, budgeting, and hygiene.
• Coordinate care with family and support systems; maintain trainings.
