Allina Health is seeking a patient-facing care coordinator to join the Courage Kenny Rehabilitation Institute team. In this role you will develop and maintain relationships with patients, guide care plans, and coordinate care with a multidisciplinary team across community-based settings.

You will work an 8-hour day shift (0.8 FTE, 64 hours per two weeks) with mileage reimbursement and no weekend work. Most patients are in the Minneapolis/West metro area, and driving is required.

Qualificações

• Must be 18 years of age with education and/or experience needed to meet required functional competencies as listed on the job description
• 0 to 2 years of experience in customer service, health care or community-based role
• High school diploma or GED
• Bachelor’s degree in health care

Responsabilidades

• Acts as the central facilitator to patients supported by the care team.
• Transitions and supports patient through all stages of care.
• Maintains and documents ongoing communications with patients and care team.
• Collects and maintains patient data and reports to facilitate care.
• Communicates with patients, families and providers to maintain coordination of care.
• Identifies and facilitates resolution to barriers.
• Develops patient-driven goals and support patients care.
• Uses electronic medical record and other electronic tools for data gathering and interpretation.
• Incorporate appropriate behavior change techniques to empower patients to be active participants in their health.
• Other duties as assigned.

Formação
