Job summary

Allina Health is a not-for-profit health system serving Minnesota and western Wisconsin. The role supports patients in their homes within the Minneapolis area, coordinating care and documenting progress in the electronic medical record.

You will collaborate with a care team to implement patient-driven goals and behavior change strategies. The position requires a valid driver’s license, 0–2 years in relevant roles, and offers an 8-hour day shift with 32 hours per two-week period and mileage

Qualifications

• Must be 18 years of age with education and/or experience needed to meet required functional competencies as listed on the job description.
• 0 to 2 years of experience in customer service, health care or community-based role.
• Requires valid driver’s license upon hire; MN and WI residents must obtain within 60 days of employment.

Responsibilities

• Acts as the central facilitator to patients supported by the care team.
• Identifies barriers to meeting health care goals and connects to resources.
• Develops patient-driven goals and supports care plans.
• Uses EMR and other electronic tools for data gathering and interpretation.
• Implements behavior change techniques to empower patients.
• Other duties as assigned.
