Allina Health is a not-for-profit health system serving Minnesota and western Wisconsin. This position provides care management support to a panel of patients requiring specialty services, coordinating with specialty and primary care teams to implement evidence-based plans of care.

The role involves assessment, planning, implementation, coordination, monitoring, and leadership within a multidisciplinary team, with potential travel to multiple locations as needed.

Qualificações

• Bachelor's degree in nursing is required.
• 2+ years of nursing experience in area of clinical specialty, care coordination or care management.

Responsabilidades

• Assessment: gather data from patient, family, and healthcare team to inform plan of care.
• Planning: develop and modify treatment plans to improve outcomes.
• Implementation and Coordination: coordinate services and resolve barriers with patient and family.
• Monitoring and Evaluation: follow patient over time and adapt the plan as needed.
• Leadership: supervise care management staff and address barriers to the plan.
• Participates in meetings and quality improvement activities.
