Job summary

Holcomb Behavioral Health System in Sussex County is seeking a Case Manager to provide community-based behavioral health services. You will assess resources to help clients live safely in the community, manage a caseload, and coordinate with multiple providers.

The role follows a Recovery Model, with focus on individualized service plans and linkages to housing, healthcare, and employment. Typical schedule is Monday–Friday, 9:00 am–5:00 pm, with on-call rotation as needed.

Qualifications

• Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education
• Be a registered nurse or have 2 years in direct client contact with 12 college credits in social sciences
• 5 years of mental health direct care experience with prior case management work (if applicable)
• Substitution of related experience/education may be considered at the discretion of the Personnel Officer

Responsibilities

• Carry a caseload of up to 30 clients
• Perform case management duties for consumers in Behavioral Health Services
• Develop individualized, strength-based service plans with measurable outcomes
• Assist consumers in obtaining housing, food, healthcare, employment, and socialization
• Provide crisis assessment and intervention when necessary
• Maintain professional relations with consumers, families, payers, and agencies
• Coordinate communication among all service providers
• Submit payroll and billing documentation in a timely manner
• Be available for an on-call rotation 24/7
