Chimes is seeking a Case Manager (Finance) to provide community-based behavioral health case management in Sussex County, DE. The role focuses on assessing resources, supporting clients to live stably in the community, and coordinating with service providers to ensure safe, effective care.

Schedule is Monday–Friday, 9:00 am–5:00 pm with on-call availability 24/7. The pay rate is $21.00/hour, with full-time benefits and a focus on recovery-based planning.

Qualifications

• Bachelor-level or RN qualification as listed in the requirements.
• Experience with case management in behavioral health preferred.
• Ability to develop measurable, strength-based service plans.

Responsibilities

• Carry a caseload of up to 30 clients.
• Provide case management services within Behavioral Health Services.
• Develop individualized service plans and monitor progress.
• Assist with housing, food, healthcare, employment, and social needs.
• Coordinate services across multiple providers and payer sources.
• Submit payroll and billing documentation accurately and timely.
• Be available for an on-call rotation 24/7.
