Job summary

Holcomb Behavioral Health Systems is seeking a Case Manager to deliver community-based behavioral health services. You will assess resources, develop strength-based plans, and help clients live safely in the community.

This full-time role operates Monday–Friday, 9:00 am–5:00 pm, in New Castle, PA, with a 24/7 on-call rotation. Compensation is $21.00 per hour, with a full benefits package and opportunities for growth.

Qualifications

• Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education.
• RN license is required.
• High school diploma with coursework in social sciences and 2 years of human services with direct client contact.
• Alternative: high school diploma with 5 years of mental health direct care experience.

Responsibilities

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers in Behavioral Health Services.
• Develop strength-based, measurable service plans with targeted outcomes.
• Assist consumers in obtaining housing, food, healthcare, employment, and socialization.
