Holcomb-Behavioral-Health-Systems in Easton, PA seeks a case manager to provide community-based behavioral health services. You will assess resources for clients to live in the community safely and stably, managing a caseload of up to 30 clients and coordinating with multiple service providers.

The role emphasizes recovery-model based practice, individualized planning, and availability for an on-call rotation 24/7.

Qualificações

• PA Driver's License with no more than three points.
• Bachelor’s degree in sociology/social work/psychology/gerontology/anthropology/political science/history/nursing/counseling/education.
• HS diploma with 12 college credits in social science and 2 years in human services with 2 years in direct client contact (1 year for Lehigh and Northampton counties).
• HS diploma with 5 years mental health direct care experience in public or private human services.

Responsabilidades

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers in Behavioral Health Services.
• Provide services based on Recovery Model principles.
• Develop individualized, strength-based service plans with measurable outcomes.
• Assist consumers in accessing mental health services and basic needs (housing, food, healthcare, employment, socialization).
• Support consumers in gaining needed resources via service plans.
• Monitor participation in the recovery plan and support services.
• Provide crisis assessment and crisis intervention when necessary.
• Maintain professional relations with consumers, families, payers, and agencies.
• Coordinate communication among service providers and support systems.
• Serve as single point of contact for linked health and social services.
• Submit timely payroll and billing documentation.
